Colorado Health Partnerships’ wants our members to get the best possible care. We want our members to have access to “Integrated Care.” Integrated Care brings together mental health, substance abuse, medical, dental and other providers to treat the whole person. Your mental health therapist will ask you about your blood pressure or your diabetes at your next appointment. Likewise, your primary care physician (PCP) now asks you about your depression and whether you are taking you mental health medication.
We want you to take advantage of all Medicaid services that will take care of the whole person. CHP provides you Medicaid Behavioral Health Services (mental health and drug/alcohol counseling). Even though CHP does not provide your health care, we want to make sure you are getting the services you need to get and stay healthy. Links to other Medicaid benefits are listed below. If you are having a hard time finding a doctor, dentist or appointment for a physical problem, you can still call the CHP Office of Member and Family Affairs. We will get you connected with the provider or services you need.
